Description: Persistent disabling low back pain can be challenging to manage when imaging findings, fear, protective behaviours, and nociplastic pain mechanisms all contribute to the presentation. This case study explores a CFT/PRT-inspired approach to clinical reasoning and rehabilitation, using behavioural experiments, pain reconceptualisation, graded exposure, and safety learning to help clinicians identify key pain drivers and support recovery from persistent protective behaviours.
